NexJ Health Coach connects people with health coaches to enable them to better manage their conditions and achieve their health and wellness goals. With NexJ Health Coach, providers can deliver personalized coaching programs that address health conditions (i.e., Type 2 diabetes or hypertension) or wellness goals (i.e., weight loss).
Patients use NexJ Health Coach to capture biometrics such as weight, blood glucose and blood pressure. They enter this data using a smartphone or tablet, and can track their daily activities such as exercise, food intake, as well as subjective information such as how they feel. Coaches review the information and can provide feedback through secure messaging. By understanding the relationship between their behaviors, their biometric measurements and how they feel, individuals will have increased motivation to drive sustained behavior change to improve their wellness.
