The NexJ Patient Portal offers an integrated web and mobile application that enables patients to access their health records and interact with their care team.
As part of NexJ Health Exchange, the NexJ Patient Portal enables patients to self-report their health measurements and observations and communicate securely with providers, enabling remote patient monitoring and minimizing unnecessary visits.
Using NexJ Patient Portal, patients can:
- Share their comprehensive electronic health record with members of their care team, including a medical summary and a history of all documents, such as discharge summaries and consult reports.
- View a history of all appointments and visits with providers and other care team members.
- View extended patient information, including a complete medical history, immunization history, and vaccinations, to better identify patients at risk for conditions.
- Co-ordinate care and enable collaboration between care team members to improve the quality of care and improve outcomes.
- Follow personalized treatment care plans and share them with their care team.
- Track progress remotely by collecting data on medications and readings from biomedical devices and sharing those results with providers.
- Communicate securely with members of their care team.
- Request consultations and referrals from members of the care team.
- Access a trusted library of health information on medical conditions and treatments.
- Receive medication and appointment reminders and track treatment progress.
- Integrate with other Personal Health Records such as Microsoft Health Vault and Dossia PHR.
